Home Health RN

Incentives: (Full-time or Part-time)

• $10,000 sign on with 2 year work commitment ($5,000 in first paycheck, remaining $5,000 after completion of 12 months of service)

Pay Range: Days: Starting Pay: $36.54

Gas mileage reimbursed at federal rate

The base pay range is a good faith representation of current potential base pay for successful applicants. . Pay is determined by factors including experience, relevant qualifications, specialty, internal equity and contracts.

• Previous RN experience is preferred.
• Previous Home Health Care experience is beneficial but not required.
• Minimum 1 year of medical-surgical preferred but not required.
• Primary Responsibilities: Planning, coordinating, providing and documenting the care of patients in their place of residence.
• Clinical practitioner who coordinates and implements patient care specific to Jefferson County.
• Ensures that compassionate, engaged care meets or exceeds quality standards.
• Care is provided in an efficient and safe manner consistent with the NYS licensure and Samaritan Home Health standard of care.
• Utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic and spiritual and educational needs of the patient/family/significant other to develop, implement and revise the plan of care under the physician’s orders. In doing so, performs accurate assessments, interprets and correlates finding, establishes plan of care and implements short and long-term goals.
• The nurse observes patient progress, records pertinent observations, implements physician and nursing treatment plans and evaluates outcomes.
• The RN recognizes their responsibilities as specified by law and abides within those responsibilities.
• Provides direct supervision to LPNs and HHAs.
• Provides on-call services as required.
• BLS required.
• Valid NYS Driver's license and proof of auto insurance.
• Must be available weekends and Holidays and provide on-call services on a rotating schedule. 

Minimum Education Requirement - Graduate of an accredited school of nursing, Current NYS RN License required.
